Name Change Form Notes

Important Points to remember:

To process your name change request, please submit the complete duly filled and signed Name Change form along with supporting documents to any ICICI Bank Branch or follow the steps mentioned below:

Steps for placing the request:

Step 1- Download Request form to download Click here - Name, Signature and MOP Change Form
Step 2- Enclose self-attested copies of Support documents as under:

Identity proof ( New Name OVD document), If new name OVD documents not available, then Marriage Certificate or Gazette copy is Mandatory for processing of Name change.

Step 3- Send the form and support documents to ICICI Bank's India address mentioned below

Mumbai – 400 072, India.

Note: You can continue with the old cheque book and debit card.

Please note that the time required for your application to reach us:
